A guest asks at 22:40 and is told someone will check. Nobody owns the reply; the morning shift never sees the thread. The review can end up carrying the answer instead.
The head housekeeper who knew which rooms hear the lift. The night manager who knew what the general manager would approve. Turnover takes it with them, and the next hire starts from nothing.
A duty manager waives, upgrades or refuses with no record of what was decided last time, or why. Tomorrow another manager decides the opposite, for the same guest.
